Emergency Medical Release:
My electronic signature below does hereby authorize the administration of emergency medical treatment for my child named on this form, during September 2024 to August 2025 for any AWANA event, including Games, Bible Quizzing and Camps.
I understand all reasonable safety precautions will be taken at all times by Christ the Rock Community Church or its agents. I will not hold Christ the Rock Community Church liable for any accident, injury or ANY disease (including COVID 19) incurred by the subject of this form. I understand that in the event medical intervention is needed every attempt will be made to contact the parent or guardian listed immediately.
