Thank you for joining us for Summer Bash! This event includes a variety of activities for children and families, including inflatable/bounce houses, a petting zoo, face painting, games, food and beverages, and other outdoor activities. Because participation in some of these activities involves inherent risks, we ask each parent or legal guardian to carefully read and sign this waiver before allowing a minor child to participate.

ASSUMPTION OF RISK

I understand that participation in Summer Bash activities may involve certain risks, including but not limited to falls, collisions, bumps, bruises, sprains, strains, cuts, allergic reactions, illness, injury, or other physical harm.

I understand that activities may include inflatable/bounce houses, outdoor games and activities, face painting, interaction with animals at the petting zoo, and consumption of food and beverages.

I voluntarily permit the participant named above to participate in these activities and understand that participation is voluntary.

I acknowledge that inflatable activities may involve risks including falling, jumping, collisions with other participants, improper landings, and other injuries. I agree to follow all posted and communicated safety rules and instructions.

I understand that interaction with animals may involve risks including scratches, bites, falls, allergic reactions, or exposure to germs or other illnesses. I understand that animals are animals and their behavior cannot always be predicted.

FOOD & ALLERGIES

I understand that food and beverages may be served at Summer Bash, including items such as hamburgers, hot dogs, popcorn, and other foods.

I understand that food may contain or come into contact with common allergens, including but not limited to milk, eggs, wheat, soy, peanuts, tree nuts, and other ingredients.

I understand that Christ the Rock Community Church cannot guarantee an allergen-free environment or prevent cross-contact between foods.

I am responsible for determining whether food and beverages are appropriate for the participant based on their allergies, dietary restrictions, or medical needs.

MEDICAL AUTHORIZATION

In the event of an emergency or injury, I authorize Christ the Rock Community Church, its staff, volunteers, and designated representatives to obtain reasonable emergency medical assistance for the participant when I cannot be reached in a timely manner.

I understand that I am responsible for any medical expenses incurred on behalf of the participant.

RELEASE OF LIABILITY

To the fullest extent permitted by applicable law, I, on behalf of myself and the participant named above, acknowledge and assume the risks associated with participation in Summer Bash and its activities.

I agree to release and hold harmless Christ the Rock Community Church, its pastors, staff, employees, volunteers, ministry leaders, representatives, and agents from claims arising from the participant's voluntary participation in the event, except to the extent caused by conduct that cannot legally be released under applicable law.

I understand that this agreement is intended to be as broad as permitted by Washington State law.

PARENT/GUARDIAN RESPONSIBILITY

I understand that Summer Bash is a family/community event and that signing this waiver does not constitute an agreement that Christ the Rock Community Church is providing childcare or individual supervision of my child.

I remain responsible for supervising my child and determining which activities are appropriate for them.

I agree that the participant will follow all reasonable safety instructions, posted rules, and directions given by event staff, volunteers, activity operators, or other authorized personnel.

PHOTO & VIDEO RELEASE

I understand that photographs and/or video may be taken during Summer Bash.

By signing below, I grant Christ the Rock Community Church permission to photograph and/or record the participant and to use those images or recordings for church-related purposes, including but not limited to the church website, social media, newsletters, promotional materials, ministry communications, and future event advertising.

I understand that no additional compensation will be provided for the use of these photographs or recordings.

ACKNOWLEDGMENT

I have read and understand this waiver. I understand the nature of the activities offered at Summer Bash and the risks associated with participation. I voluntarily give permission for the participant named above to participate.

I certify that I am the parent or legal guardian of the participant or otherwise have legal authority to sign this form on their behalf.
